	Introduction

	Below you will find the forms and explanations necessary to get legal paperwork. All requests and forms should be returned to: FireAngelMysti.

	Last Rites Testament

	Last Rites Testament is a verbal contract involving someone speaking prior to their death in the presence of at least one other person. The Last Rites Testament is for those who have not yet obtained a written will. To authenticate a last rites testament with the Minister in charge of Legal Services, send in the full orb (AOL/AIM Log) documenting the scene where the individual has entered into that aforementioned verbal contract, including the witness(es) and the subsequent death. Please note that if you would like the death announced in the newsletter you must state so in this submitted document.

	Written Will

	A written will may be changed at anytime only by the individual who wrote the will. Testaments of the written word may include properties, gold, personal effects, guild rank and custody of children. Because Parliament and Trinity positions are by a vote of majority, these positions may not be willed to an individual to receive. A strong verse of recommendation may be forwarded in with the will to attest to the ability of an individual for a better running vote however, like a letter of recommendation. To authenticate a will on record with the Minister in charge of Legal Services, fill out the form below. Wills must be written by the member the will is being written for and signed by at least two witnesses before being sent to the Minister in charge of Legal Services and the Trinity for approval.

	Marriage Certificate

	A Certificate of Marriage may be requested with the form below. The priest/ess performing the ceremony can be given credit for a ceremony if the orb is turned in to the Healer Ministry, independent of the below form.

Baptism Form:

	Baptism

	A baptism, to whatever faith, is a part of anointing a child or person into the following of a deity or religion. Usually the ceremony should be operated by a priest or priestess of the faith that the child is being baptized into. For baptisms well performed, should the priest/ess wish to gain credit, they should send the orb in to the Healer Ministry along with this form.

	Upon the receipt of this form you will receive a certificate of religion membership for the child.

Eulogy Form:

	Eulogy

	There are few words that can be said to fully express the loss one feels when a loved one has departed permanently from the realms. As it is an honor to the departed and the family and friends involved for one individual to be given the right of Eulogy. If the Priest/ess delivering the Eulogy wishes to get credit for their work they must send this form in with the orb to the Healing Ministry as well.

	Upon the receipt of this form you will receive a death certificate for the person in question.
